This form is to request water services be disconnected. In order to submit this form you will be required to enter your account number. If you do not have your account number, you will need to contact the office. Requests for termination will be processed during normal business hours. Once your request has been completed you will be sent a confirmation number via email. If you do not receive a confirmation number, your request has not been processed. Please contact the office if you do not receive your confirmation number.

We cannot accept requests any further in advance than 5 business days prior.

Once a request is received, the termination will occur on the selected date between 8am and 5pm. Requests can be scheduled Monday through Friday.
