  2. What to Bring to Your Appointment
    Bring Baby. Hospital discharge paperwork. WIC folder. Mother's Vertification of Facts, Crib card, ID bracelet. Proof of residence. Proof of income: LES (current month), pay stubs (last 30 days), medicaid card, food stamp letter

  5. Proof of Identification for Parent/Guardian
    Photo ID (driver's license, work ID, military ID, government ID, passport). Medicaid card. LES. Marriage license less than 60 days old. Bill or account (less than 60 days old). Social Security card. Blank check. ATM/debit card. Correspondence less than 60 days old. Food stamp (FNS) letter. Income tax filing for most recent year. Voter registration card.
  6. Proof of Identification for Infant
    Mother's Verification of Facts. Immunization record. Social security card. Crib card or hospital ID bracelet ONLY if it contains infant's name.
  7. Proof of Residence - Physical Address Required
    Photo ID (driver's license, work ID, military ID, government ID, passport). Medicaid card. LES. Marriage license less than 60 days old. Bill or account statement (less than 60 days old). Property tax statement. Immunization record (infants only). Foster care placement letter.
  8. Proof of Income
    Medicaid card. Food stamp (FNS) letter showing certification dates. LES (current month). Paycheck stubs (last 30 days). Signed statement from employer indicating gross income earnings for a specified period. W-2 or tax return from most recent year for self employed. Cash income. Foster care placement letter. Alimony or child support. Retirement or pension. Worker's compensation. Unemployment letter. Supplemental security income. Social security benefits. Veteran's Administration benefits. Money from individuals not living in the household. Student grants.
